At Integrated Care Professionals, our nurse practitioners truly become a part of their facilities team. We collaborate with everyone involved in our residents care, including dietary wound care, the Director of Nursing, the Executive Director, as well as physical therapy, occupational therapy and speech each therapy on a daily basis to proactively diagnose and treat changes in resident conditions before they become emergent. We work with the individual facilities to attain quality measures to also improve their star rating by strategically collaborating daily with MDS. Collaboration is essential to providing consistent and accurate diagnoses in order to help facilities achieve maximum reimbursement. Under the new PDPM payment model, residents benefit by having a full time onsite provider who is following their care closely. They experience fewer hospital visits, have access to better chronic care management, and often reduced medication. This ultimately leads to a higher quality of life for our patients, which is some something we here at ICP strive for every day.
