Hey, I’m Ashley Kirk.

And today we’re going to be talking about integrated care professionals and what we specialize in. We do everything. We do wound care, we do psychiatric care. We know how to work with psychologists. Psychiatrist in managing those medications that those people are on. We reduce antipsychotics in these facilities, which is something that CMS really wants us to do. We work to reduce narcotics in the facility, work to reduce polypharmacy. Polypharmacy a lot of times, is a really big issue that a lot of patients are facing coming in the home. Another thing that I really like seeing is our providers are able to dig deep and really dig through hospital discharge papers and make sure that there’s not a medicine that’s on the list that’s incorrect. They’re able to take care of any medication changes that need to happen on the front end when they admit rather than four or five days later. That’s just been a really nice thing for a lot of our homes because a lot of times they do come from the hospital with different medications that maybe they don’t need to be on.

That’s a good point. Oneof our providers actually kept track. Of some of the medication changes that were going on with some of his new admissions and noticed that on all of his new admissions from the hospital, he found, I think 75% of all of those admissions had some form of medication error. And so it was because of his diligence and him talking to the providers and their families and finding out what their actual treatment regimen was supposed to be and not what basically they were discharged on basically prevented a pretty serious adverse side effect from some medications they weren’t supposed supposed to be on. And that’s the truth. I got proof!
